Insurance Information

Canton ENT accepts most insurance plans.   However patients still have certain responsibilities.   Prior to office visits, patients should make sure that they have the basic understanding of their insurance policy.

• At the time of visit, patients are required to pay any co-pay, unmet deductible, or co-insurance.
• Bring current insurance card for each visit.   Without insurance card, patients are responsible for full payment.
• Please notify our office of any change in your coverage.   We must be notified if you have your insurance company changed or if you have received a new card from the same insurance company.

Our office will file all charges with the insurance company.   Canton will also provide the insurance company with all the medical information needed in order to accurately process your claim.
